WHAT IS CHILD HEALTH PLAN PLUS (CHP+)

CHP+ is low-cost health and dental insurance for Colorado children. It’s for families that make too much money to join Medicaid but not enough to afford private insurance. In general, a family of four with an income up to $55,000 a year can get theri kids covered.


WHO QUALIFIES FOR CHP+?
Children may qualify if:

  • They are 18 or younger
  • They live in Colorado
  • They don’t qualify for Medicaid
  • They don’t have other health insurance
  • Their families meet the CHP+ income limits
  • They are a U.S. citizen, a legal permanent U.S. resident for at least 5 years, an asylee or refugee


ARE THERE SPECIAL SERVICES FOR PREGNANT WOMEN?
A pregnant woman can get health care right away while she waits for CHP+ to look at her application. This is called Presumptive Eligibility.

HOW MUCH DOES CHP+ COST?
Depending on your family size and income, you may need to pay an enrollment fee once each year, and co-pays (what you pay each time you get a service).

  • Some families pay nothing
  • Some families pay up to $35 per year (no matter how many children there are in the family) and $2 or $5 co-pays for some medical and dental services
  • Pregnant women have no co-pays


Children in CHP+ get their health services in an HMO (Health Maintenance Organization). An HMO is a group of doctors, nurses, hospitals and other providers who work together to give children in CHP+ health care. Members must visit a doctor who is in their CHP+ HMO.

 

WHAT IS FAMILY MEDICAID?

Colorado Family Medicaid is health insurance for low income families.

WHO QUALIFIES FOR FAMILY MEDICAID?
To qualify, families or individuals must:
  • Have a child, be pregnant or
  • be 18 years of age or younger
  • Live in Colorado
  • Meet the Medicaid income limits
  • Be a U.S. citizen, a legal permanent U.S. resident for at least 5 years, an asylee, a refugee or a non- citizen with a qualifying emergency

WHAT DOES MEDICAID COST?

Most Medicaid services are free, but there is a small co-pay (what you pay each time you get a service) for some services.
Children 18 and under have no co-pays for any services
Pregnant women (any age) have no co-pays for any services
Adults 19 and over have small co-pays for doctor visits, prescription drugs and hospital stays
