Health Center

The Northwest Colorado Community Health Center (formerly the Moffat County Care Clinic), is a primary care clinic serving Moffat County. We deliver quality and affordable health care to Moffat County residents. The clinic has been in operation for many years has been operated by the Northwest Colorado Visiting Nurse Association since 2005.

Our success is our partnerships with our patients and community. We are proud of our achievements and we look forward to continuing to build our capacity in order to serve our growing community.
