Pulmonary Health Program |
This program is made possible by the Colorado Department of Health and Environment in conjunction with Northwest Colorado VNA. The goal of this program is to diagnose, educate and refer patients in Routt, Moffat and Jackson counties with asthma, COPD (chronic obstructive pulmonary disease) and other pulmonary diseases. Early screening, education, and treatment will be utilized to help patients manage their pulmonary diseases, improving their quality of life and decreasing the need for hospitalizations. This program is open to patients of all ages and genders.
The
Northwest Colorado VNA’s Pulmonary Health Program offers FREE spirometry
(breathing screenings) to anyone ages 7 and up in Routt, Jackson, and Moffat
counties. Patients who are uninsured/underinsured may receive prescription
assistance, ongoing disease management and education by a certified asthma
educator, and pulmonary function testing based on financial need. Please call
