HCP-Health Care Program for Children with Special Needs |
The Health Care Program for Children with Special Needs (HCP) is a unique resource for families, health care providers and communities. Our goal is to help improve the health, development and well-being of Colorado’s children with special health needs and their families. HCP serves children from birth to age 21 that have, or are at risk for, serious physical, behavioral or emotional conditions. HCP has regional offices and partners throughout Colorado.

Families
Maybe you just learned that your child has special health care needs. Or maybe you have been trying to help your child for a long time. HCP will stand by you. At HCP, professionals and families work as a team. HCP provides information, referral to services and support. Families make choices and take action. Together, we help children with special health needs get what they need and grow to be their healthiest, as well as reach the full potential of their independence.
The HCP Team is available to provide care coordination to families. Care coordination is the facilitation of access to and coordination of medical and social support services. It includes both public and private covered and non-covered services resulting in improved health and quality of life.
HCP provides care coordination through the Colorado TBI Program for children from birth to age 21 who have sustained a traumatic brain injury. Families interested in TBI Care Coordination first apply to the Brain Injury Association of Colorado. Applications available at www.biacolorado.org
Service Providers:
Maybe you are interested in learning more about caring for children with special health needs. Or, you are highly experienced and would like to share your knowledge with others. HCP will listen to you, talk with you about solutions and directions, connect you with what’s needed, provide information, data and support, and introduce you to other professionals that share your interests.
Communities:
Maybe you are interested in building your community’s capacity to help children with special health needs. Or, you want to network with other community leaders and health professionals. You may be looking for help with a specific issue or challenge, or to develop new policy and new resources. HCP seeks to ensure statewide access to integrated, family-centered, culturally competent, and community-based programs and services.
